Anatomy of Bhagandar (Piles or Hemorrhoid)

The popular Ayurvedic technique kshar sutra is related with the disease bhagandar. In modern science it is referred as piles or Hemorrhoids. Here in this article I have tried to give concept of different types of piles.

  1. Internal Piles or true piles- In this piles there is a saccular dialation of internal rectal venous plexus which occur above the pectinate line . And this type of Piles are painless.
    a. Primary piles- This occur in 3,7and 11’0 clock position of anal wall which are formed by enlargement of 3 main radicles of superior rectal vein which lie in the anal column which occupy left lateral, right posterior, and right anterior position.
    b. varicosities in other positions of lumen are secondary piles
  2. External piles or False piles- This occurs below the pectinate line and are very painful . In this piles there is no bleeding or staining in stool
  3. Fistula in ano- It is caused by spontaneous rupture of an absess around anus or many follow surgical drainage of abscess. Most of these abscesses are formed by small vestigial glands opening into the anal sinus.
  4. Rectal continence – Rectal continence depends solely on the anorectal ring. Damage of this ring results in rectal incontinence. Surgeon has to carefully protect the anorectal ring while operating the region.

Furthermore in Ayurvedic concept, one must protect the marma in the anal canal i.e Gud marma .  The surgery in ayurveda is done by a thread (sutra), which is combined with alkali (kshar). This medicated thread is tied around.

      well there are different stage of bhagandar clinically,
      first When Pus arrives in the inflammation – medicine like Navkartik Guggul is best used
      Next, When Pus has not arrived yet – medicine like Ravitandav Ras – 1 Masha, Ras Manikya – 1 Ratti, and Ras Sindoor – 1 Ratti (all of them mixed with honey)

      Also below drugs can be used
      ChandraPrabha Vati 1-1 (morning-evening)
      Kanchnar Guggul 2-2 (morning-evening)
      triphala (with milk) 1-1 (morning-evening)

      however it is wise to visit a doctor.
